FLU SHOTS DEPARTMENT OF HEALTH FOR ALL STUDENTS
The Florida Department of Health and Hillsborough County Public Schools, will be teaming up to provide flu shots to students, families and district staff. Our plan is to host clinics at five schools located throughout our community during the month of December.
This is a great opportunity for you and your family to receive a vaccination. Flu season runs from October through May and the Centers for Disease Control says vaccinations are the best way to protect you from getting sick. The Florida Department of Health – Hillsborough is offering this service with no out of pocket expenses to our families or staff. Your insurance may be billed if your policy will cover the shot. Students who elect to participate must be accompanied by a parent or legal guardian in order to receive a flu shot.
Vaccinations will be provided by appointment only. Following CDC guidelines, the clinics will be set up in a drive-through format and participants will be directed to stay in their vehicles. Families can go to any of the locations on the dates and times listed below.
Each person who is receiving a flu shot MUST:
• Be pre-registered and receive an appointment time by calling (813) 307-8085, Monday through Friday from 8-5 p.m.
• Pre-register at least 24 hours in advance of the clinic you plan to attend.
• Download and fill out this permission form https://bit.ly/ ShotFormEnglish or https://bit.ly/ShotFormSpanish and bring to the clinic.
• Arrive at your scheduled appointment time or you may not be able to receive the vaccination.
• Remain in your car. NO walk-ups will be permitted.
• All persons in the vehicle must wear a face covering or mask.
• Anyone in your household is eligible to receive a flu shot through this program.
